Three-session training: one-page final-report template

Illustrative reporting template. No results are represented as measured. Replace bracketed fields only with source-backed counts or narrative; suppress identifying detail.

Cohort and delivery. Audience: [role mix, if safe to report]. Session dates: [date 1], [date 2], [date 3]. Registered: [n]. Attendance: Session 1 [n], Session 2 [n], Session 3 [n]. These are attendance counts, not unique people unless registration records permit that calculation. Delivery changes or access adjustments: [what changed and why].

Measure Baseline End check Interpretation
Responses received [n] of [eligible n] [n] of [eligible n] Report response rates and any missing or unmatched answers.
Paired responses, if linkage was consented and possible [n] [same n] Compare only paired responses; do not treat different respondents as a before/after cohort.
Met the pre-set task rubric [n]/[denominator] [n]/[denominator] State rubric, task differences and uncertainty; include counts as well as percentages.
Correctly stopped or escalated a risky case [n]/[denominator] [n]/[denominator] Report this separately; rejecting every case is not proficiency.

What participants demonstrated. [Two or three observed task decisions, each tied to a worksheet or check. Separate facilitator observation from participant self-report.]

Feedback and access. Useful: [theme and count/source]. Unclear: [theme and count/source]. Barriers: [theme and count/source]. Accessibility adjustments used: [aggregate description only]. Do not publish a recognizable quote or small subgroup detail without permission.

Limits. With a small or self-selected cohort, a few responses can move a percentage substantially. Missing or unpaired checks can bias a before/after comparison. A short end check measures performance on the training task, not sustained workplace behaviour or causal impact. No finding about a subgroup should be made from a revealing cell; use counts and qualitative context where appropriate.

Recommendations. 1. [Immediate improvement, owner and date.] 2. [Practice or supervisor support, owner and date.] 3. [A bounded follow-up measure, including a no-action option.] Any use beyond the training exercise needs the organization’s own privacy, safeguarding and operational approval.

Transfer (30 days, self-reported). Response rate: [n]/[eligible n]. Item response distributions and valid n: [summary]. N/A and missing responses: [counts]. Top barriers (counts): [list]. Support themes (aggregate): [brief]. Source: [transfer-follow-up instrument version/date].

Evidence register. Attendance source: [file/date]. Rubric and answer key: [version]. Feedback instrument: [version]. Transfer instrument: [version]. Aggregation method and exclusions: [brief note]. Reviewer/acceptance: [name or role/date].
